Fragrance Policy
The rising incidence of upper respiratory conditions, asthma and migraines combined with increased use of fragrance products has led us to adopt a fragrance free policy. We ask that you refrain from applying colognes, perfumes or scented lotions on days that you have a physical therapy appointment as the use of these products may create health problems for your therapist or other patients. We thank you for your cooperation.
Clothing
Please bring shorts and a t-shirt with you if you have a lower back or lower extremity problem. Females with a low back, neck, upper back or upper extremity problem may bring a sports bra, halter top, bathing suit top or choose to use a provided gown.
Insurance Coverage
In Washington, patients have direct access to the services of physical therapy without the need for a prescription from a physician or
non-physician practitioner. However, for physical therapy to be payable by most insurance companies we advise you to obtain a prescription, referral or pre-authorization from your physician or
non-physician practitioner.
Star Physical Therapy is a preferred provider for most of the major insurance carriers in Pierce and King County. Each insurance company has different coverage for physical therapy. There may be limitations, deductibles, or co-payments that must be met by the patient. Most managed care plans require prior authorization by your primary care physician. We will contact your insurance company before your treatment but advise you to do the same to be sure of your coverage for physical therapy services.
Medicare patients
are welcome. Star Physical Therapy accepts assignment up to
the maximum Medicare allowables.
Motor Vehicle
Accidents: Personal Injury Protection coverage may cover your
medical bills that are a result of an accident. Consult your
insurance company to determine if coverage is available.
Managed Care
Insurance: Prior authorization is required by your primary
care physician.
